Short-term Medical Treatment in Taiwan for Foreign Nationals

(1) Visa Application Form — Must be filled out via the "Online Visa Application Form (https://visawebapp.boca.gov.tw/BOCA_EVISA/)." Print out the barcode form and sign in person.
(2) Two 2-inch color photos taken within the last 6 months — white background required.
(3) Original passport and a photocopy — must be valid for at least 6 months and contain blank pages.
(4) Supporting Documents —
1. Proof of financial sufficiency (e.g., bank statements or a payment agreement with a Taiwan medical institution).
2. Diagnostic certificates from a foreign hospital.
3. Medical treatment schedule and statement from a domestic medical institution in Taiwan.
(5) Round-trip airline ticket — air tickets, e-tickets, or a travel agency certificate.
(6) Other documents required on a case-by-case basis.

(1) Prepare Required Documents and Visa Fees — The applicant must gather all required documents and fees to apply for a visitor visa at an overseas mission of the Republic of China (Taiwan).

(2) Potential Interview Requirement — When necessary, the overseas mission will require the applicant to attend an interview.

(1) Visa issuance is an act of national sovereignty; the government of the Republic of China (Taiwan) retains the right to deny a visa without providing any reason. Regardless of whether a visa is issued or not, the visa processing fees paid are non-refundable by law.

(2) For visitor visa fees and the reciprocal processing fees for U.S. citizens, please refer to the "Standard Fees for Foreign Passport Visas."

(3) The categories of diseases eligible for visa applications to receive medical services in Taiwan shall be subject to the "List of Diseases for Foreign Nationals and Mainland Area Residents Eligible for Medical Services in the Taiwan Area (PDF File)" announced by the Ministry of Health and Welfare.

(4) Domestic medical institutions are strictly limited to the "Member Institutions of the Medical Services Internationalization Program" announced by the Ministry of Health and Welfare.

(5) In principle, accompanying caregivers are limited to a maximum of two individuals, who must be the patient's spouse or relatives within the third degree of kinship. When necessary, up to two healthcare professionals from the patient's country of residence may be added to accompany them for medical care. Applicants may apply for a visa to Taiwan for the purpose of accompanying the patient by presenting proof of relative relationship or a deployment certificate issued by the healthcare professional's employer.

(6) Major Emergency Medical Visa Applications: These applications can be submitted to the Bureau of Consular Affairs of the Ministry of Foreign Affairs directly by a medical institution in Taiwan.

Medical Care and Accompanying Caregivers for Mainland Area Residents in Taiwan

(1) Residents of the Mainland Area suffering from diseases announced by the central competent health and welfare authority as eligible to receive medical services in Taiwan may apply to enter Taiwan for medical treatment, which must be received at medical institutions announced by the central competent health and welfare authority.

(2) For individuals applying to come to Taiwan under the preceding paragraph, up to two companions who are their spouse or relatives within the third degree of kinship may apply to accompany them. When necessary, up to two healthcare professionals from the Mainland Area may also apply to accompany them for medical care.

(1) Application Form for Residents of the Mainland Area Entering/Leaving the Taiwan Area.
Must be filled out truthfully, with a 2-inch color photo with a white background taken within the past 2 years securely attached. For photo specifications, please refer to the National Immigration Agency, Ministry of the Interior website.

(2) Photocopy of the Mainland Area Resident Identity Card, or Photocopies of Other Certifications/Documents Sufficient to Prove Identity.

(3) Notarized Certificate of Kinship or Other Documents Sufficient to Prove Relationship between the applicant and the accompanying relatives, verified and authenticated by an institution established or designated, or a private organization entrusted by the Executive Yuan (such as the Straits Exchange Foundation).

(4) Original Application Form for Medical Institutions Applying on Behalf of Mainland Area Residents to Receive Medical Services in Taiwan — stating the institution's willingness to provide medical services and to bear the relevant expenses incurred if the applicant is subject to forced deportation and detention due to violations of regulations or abnormal behavior.

(5) Medical Plan and Treatment Schedule.

The guarantor must provide:

1. A signed Letter of Guarantee. 2. The original and a photocopy of the guarantor's National Identity Card (the original will be returned after verification). 3. The Letter of Guarantee must be verified by the local service centers of the National Immigration Agency in each county/city. (7) For Applicants Residing in Overseas Areas, Hong Kong, or Macau — An additional re-entry visa for the overseas area, an overseas residence permit, or a Hong Kong/Macau Identity Card must be attached.

(8) Certificate Fee of NT$600.

(9) Other Relevant Supporting Documents.

1. Accompanying healthcare professionals: A statement explaining the necessity of traveling to Taiwan, a roster, medical professional certifications, and the medical plan and treatment schedule of the person being cared for.
2. Other supporting documents required by relevant authorities.

Applications must be submitted on behalf of the applicant by a medical institution that meets the criteria announced by the central competent health and welfare authority to the local service centers of the National Immigration Agency in each county/city.

(1) Issuance of a 1-year to 3-year multiple-entry permit that requires endorsement for each entry. The holder can enter and exit the country after obtaining an endorsement within the validity period; the validity of each endorsement is 3 months starting from the day following the endorsement. However, it shall not exceed the validity period of the multiple-entry permit itself (the duration of stay will be granted based on the total medical treatment duration specified in the medical plan and treatment schedule). This also applies to patients returning for follow-up consultations.

(2) If the requested medical treatment is short-term and can be completed in a single visit, a single-entry permit will be issued based on the relevant certification provided by the medical institution stating that no follow-up consultation is required.

(3) Accompanying relatives providing care will be issued the same type of permit and validity period as the applicant; accompanying healthcare professionals providing care will be issued a single-entry permit.

(1) For residents of the Mainland Area applying to come to Taiwan to receive medical services, the duration of stay shall not exceed 3 months. When necessary, it may be extended for up to 3 months per extension. For patients returning for follow-up consultations, an additional 5 days will be added to their duration of stay in Taiwan, provided that the total stay does not exceed the 3-month validity limit.

(2) Accompanying relatives providing care will be granted the same duration of stay as the applicant. Accompanying healthcare professionals providing care will be granted a duration of stay determined based on their activity itinerary, which shall not exceed 3 months.

The applicant must prepare the following documents within 30 days prior to the expiration of the duration of stay:

(1) Application Form for Extension. The Letter of Guarantee must be verified by the local service centers of the National Immigration Agency in each county/city. (2) Entry/Exit Permit.

(3) Valid Travel Permit for Mainland Residents to Taiwan or Passport. If the validity of the permit/passport is insufficient, the stay can only be extended to 1 month prior to the expiration date of that document.

(4) Supporting Documentation Issued by the Attending Medical Institution Certifying the Definitive Necessity of an Extension, and the Application Form for Medical Institutions Applying on Behalf of Mainland Area Residents to Receive Medical Services in Taiwan.

(5) Extension Fee of NT$300 — Applications must be submitted directly to the local service centers of the National Immigration Agency in each county/city (mail-in applications are not accepted).

Health Examinations and Aesthetic Medicine for Mainland Area Residents in Taiwan

Medical institutions announced by the central competent health and welfare authority that are authorized to apply for health examinations and aesthetic medicine in Taiwan on behalf of residents of the Mainland Area.

Residents of the Mainland Area who meet one of the following conditions may apply for permission to enter Taiwan to receive health examination or aesthetic medicine services:

(1) Must be at least 20 years of age and possess savings equivalent to NT$200,000 or more, hold a bank-issued gold card, or have an annual wage income equivalent to NT$500,000 or more.

(2) Their lineal blood relatives and spouse may apply concurrently. Individuals under paragraph (2) who are under 18 years of age are exempt from undergoing the health examination or aesthetic medicine services.

(1) Application Form — Applicant data must be registered via the National Immigration Agency (hereinafter referred to as the NIA) "Online Application and License Issuance Management System for Short-term Entry of Mainland, Hong Kong, and Macau Residents (https://mt.immigration.gov.tw/PB/login/auth?applyType=MC)" (hereinafter referred to as the Online System). Printing a paper copy is not required.

(2) Mainland Area Resident Identity Card (hereinafter referred to as the Resident ID Card), Other Passports/Permits, or Documents Sufficient to Prove Identity.
Minor applicants who have not yet obtained a Resident ID Card must attach a Household Registration Card (Permanent Population Registration Card) of the Mainland Area.

(3) 2-inch Color Photo with a White Background Taken Within the Last 2 Years.
The attached photo must comply with the specifications for Taiwan National Identity Cards and must be identifiable as the same person as shown in the held Resident ID Card and Travel Permit. Applications that fail to provide photos according to these regulations will not be accepted.

(4) Qualification and Supporting Documents:

— Proof of Savings of NT$200,000 (approximately RMB 50,000) or More: A bank statement issued by a bank or financial institution within the last 3 months, showing a deposit duration of at least 1 month, or bank account transaction history statements within the last 1 month (must bear the official stamp of the bank or financial institution). — A Credit Card of Gold Tier or Above Issued by a Mainland Bank or Financial Institution: Supporting documents or color scanned electronic files of credit cards such as Gold, Platinum, or Infinite cards. — Proof of an Annual Salary Income of NT$500,000 (approximately RMB 125,000) Within the Last 3 Months: The annual salary income certificate must bear the company's official stamp, issuance date, and state a tenure of employment of more than 1 year. — Verification Exemption: For Mainland Area residents whose applications are submitted by a medical institution, verification is exempted if they have not been involved in any criminal cases within the last 6 months. If the applicant has been involved in any criminal case in Taiwan, all financial proof (including bank statements, account transaction history, credit cards of gold tier or above, and annual salary certificates) must be verified/authenticated by the Straits Exchange Foundation (SEF). — Proof of Kinship for Accompanying Lineal Blood Relatives and Spouse: Documents such as a birth certificate, marriage certificate, kinship certificate issued and stamped by the Public Security Bureau, or a full Household Register (Hukou). (5) Health Examination or Aesthetic Medicine Service Proposal/Plan. Applicants reapplying within 6 months of departure must attach an explanatory statement from the medical institution clarifying the distinction between the previous and current examination items.

(6) For Applicants Residing in Overseas Areas, Hong Kong, or Macau — A re-entry visa for the overseas area, an overseas residence permit, or a Hong Kong/Macau Identity Card must be additionally attached.

(7) Power of Attorney / Letter of Entrustment — A Power of Attorney must be attached if the application is submitted by a travel agency on behalf of the applicant.

(8) Payment Receipt Issued by the Medical Institution: The total amount for the health examination or aesthetic medicine received by the Mainland Area resident specified on the receipt must comply with the following regulations:

— Issued Under an Individual Name: If the medical institution issuing the receipt is a Medical Center, the amount per person shall not be lower than NT$20,000; if it is a non-Medical Center, the amount per person shall not be lower than NT$15,000.
— Issued to a Handling Travel Agency for Forwarding: The total amount on the receipt shall be calculated by multiplying the charging standards by the number of applicants. The receipt must clearly state the individuals receiving the service (e.g., "Chen XX and X others") and attach a complete roster of all personnel.
(9) Other Supporting Documents.

(10) When reviewing health examination and aesthetic medicine cases, if the NIA service center deems the attached qualification documents to be doubtful, it may require that the qualification documents be verified/authenticated by the Straits Exchange Foundation (SEF).

(1) Application Procedures

Medical Institution Applications:

1. The medical institution shall apply by logging into the NIA's "Online Application System for Mainland Area Residents Entering the Taiwan Area for Health Examinations and Aesthetic Medicine" using an online identity certificate (digital certificate) or a username and password.
2. After verifying that the applicant's required documents are correct, the medical institution shall scan the original documents into electronic files and upload them to the NIA's online system. Once the application is reviewed and approved, and the medical institution completes the payment, the electronic file of the Entry/Exit Permit (in PDF format) can be downloaded.

Travel Agency Applications:

Travel agencies that have been approved by the Tourism Bureau, Ministry of Transportation and Communications in accordance with Article 10 of the Regulations on Permitting Residents of the Mainland Area to Engage in Tourism Activities in Taiwan, and whose approval has not been revoked by the Bureau, nor have they voluntarily suspended their handling of Taiwan-travel-related business for Mainland Area residents, may apply via the NIA's online system on behalf of the applicant.

(2) License Issuance Methods

1. The medical institution downloads the electronic file of the Entry/Exit Permit, prints it out, and sends it to the applicant to hold for entry.
2. The medical institution downloads the electronic file of the Entry/Exit Permit and transmits it to the applicant, and the applicant prints out the Entry/Exit Permit by themselves.

(3) Important Notes

1. To maintain the print quality of the Entry/Exit Permit, the latest version of PDF reader software should be used to read and print the electronic file.
2. When printing the Entry/Exit Permit issued by the NIA's online system, please use A4 white paper and print in full-bleed color using a laser printer.

(1) Permit Fee of NT$600: The fee is NT$600 per person and must be paid at the time of application via the online system.

(2) Type and Validity of the Issued Permit: A single-entry permit will be issued, with a validity period of 3 months starting from the date of issuance.

(3) Duration of Stay in Taiwan Shall Not Exceed 15 Days: For residents of the Mainland Area applying to come to Taiwan for health examinations or aesthetic medicine, their duration of stay will be approved by the competent authority based on their activity itinerary. The total duration of stay in Taiwan per entry shall not exceed 15 days.

(4) Loss of Entry/Exit Permit Prior to Entry: If the Entry/Exit Permit is lost before entering the country, it must be handled in accordance with the issuance methods specified in Paragraph (2) of Point IV. Applicants shall not apply to the NIA for a replacement after arriving at an airport or port. Those who fail to comply will be denied entry in accordance with Subparagraph 1, Paragraph 3, Article 13 of the "Regulations on Permitting Residents of the Mainland Area to Enter the Taiwan Area."

(5) Loss of Entry/Exit Permit After Entry: If the Entry/Exit Permit is lost after entering the country, the medical institution or the entrusted travel agency shall apply for a replacement and print the permit via the NIA's online system. If the loss of the permit is discovered only at the time of departure at an airport or port, the applicant may apply for a replacement at the Border Affairs Corps of the NIA at the respective airport or port.
